What are the required documents for changing the Delta name?

Depending on the reason for the name change, the following documents must be submitted:

  • A government-issued ID or passport provides the correct name for validation.
  • Marriage Certificate: This document is required for name corrections related to marriage.
  • Divorce Decree: When resuming your maiden name after divorce, you will need to have this document.
  • Court Orders: Legal name changes generally require court orders.
  • Notarized Statement: Required occasionally to confirm the reason for changing a name.
